A C-SNP (Chronic Condition Special Needs Plan) is generally for people with qualifying health conditions such as diabetes or other chronic conditions the plan is designed around.

A D-SNP (Dual Eligible Special Needs Plan) is for people who have both Medicare and Medicaid.

On extras, Diondre notes many of these plans can include dental, vision, hearing, transportation, and over-the-counter benefits. C-SNPs use a similar benefit toolbox, but the plan is usually built around the chronic condition. Not every C-SNP is the same.
